WAVE PRIVACY STATEMENT
This Privacy Statement is effective for all users of eyefive.net (WAVE or our) and our related websites, including Customers, Affiliates and Newsletter Subscribers (Users). The purpose of this Privacy Statement is to provide you with WAVEs information collection practices (including the information WAVE collects, how it is collected, and the circumstances under which the information is disclosed), and to demonstrate WAVEs firm commitment to privacy and adherence to the North Carolina Online Privacy Protection Act. This Privacy Statement is effective this 1st day of July, 2006.
Our site uses an online secure order form. When a Users order is placed through our secure online server, or a User sends WAVE an email, WAVE collects confidential customer information. When a User sends an email (as distinguished from a secure order form, which is encrypted), it is not necessarily secure and is not encrypted. Information collected from affiliates and newsletter subscribers may not be encrypted or maintained on a secured site. All of the customer data WAVE collects is protected against unauthorized access.
INFORMATION COLLECTION & USE
WAVE is the sole owner of the information collected on this site. WAVE will not sell, share, or rent this information to others in ways different from what is disclosed in this statement. WAVE collects information from our users at several different points on our website.
To use this web site, a User must first complete the registration form. During registration a User is required to provide contact information (including name and email address). This information is used to contact the User about the services on our site for which they have expressed interest. It is optional for the user to provide demographic information (such as income level and gender), and unique identifiers (such as social security number), but disclosure of such information is encouraged so we can provide a more personalized experience the WAVE site.
WAVE requests information from the User on an order form. The information that is collected to process the order may include contact information (name, shipping address, telephone number, fax number and/or email address) and financial information (credit card number, name on account, expiration date). This information is used for billing purposes and to fill customers orders. This contact information is used to contact the User.
WAVE uses IP addresses to analyze trends, administer the site, track users movement, and gather broad demographic information for aggregate use. IP addresses are not linked to personally identifiable information.
WAVE may share aggregated demographic information with WAVE partners and advertisers. This is not linked to any personal information that can identify any individual person.
WAVE may use several credit card processing companies and banks to bill users for goods and services. These companies do not retain, share, store or use personally identifiable information for any secondary purposes.
WAVE may partner with another party to provide specific services. When the user signs up for these services, WAVE may share names, or other contact information that is necessary for the third party to provide these services. These parties are not allowed to use personally identifiable information except for the purpose of providing these services.
WAVE may release account information when WAVE believes, in good faith, that such release is reasonably necessary to comply with law, prevent fraud, enforce or apply the terms of any of our user agreements or to otherwise protect the rights, property and safety of WAVE, our customers or other third parties.
This web site contains links to other sites. Please be aware that WAVE is not responsible for the privacy practices of such other sites. WAVE encourages its Users to be aware when they leave the WAVE site and to read the privacy statements of each and every web site that collects personally identifiable information. This privacy statement applies solely to information collected by this Web site.
If a User wishes to subscribe to our newsletter, we ask for contact information such as name and email address. Newsletter subscriber information is not maintained on a secure site.
SURVEYS & CONTESTS
From time-to-time WAVE requests information from Users via surveys or contests. Participation in these surveys or contests is completely voluntary and the User therefore has a choice whether or not to disclose this information. Information requested may include contact information (such as name and shipping address), and demographic information (such as zip code, age level). Contact information will be used to notify the winners and award prizes. Survey information will be used for purposes of monitoring or improving the use and satisfaction of this site.
If a User elects to use the WAVE referral service for informing a friend about our site, WAVE asks for the friends name and email address. WAVE will automatically send the friend a one-time email inviting them to visit the site. WAVE stores this information for the sole purpose of sending this one-time email. The friend may contact WAVE at firstname.lastname@example.org to request the removal of this information from their database.
This web site takes every precaution to protect Users information. When users submit sensitive information via the website, the information is protected both online and off-Line. When the EYEFIVE registration/order form asks users to enter sensitive information (such as credit card number and/or social security number), that information is encrypted and is protected with encryption software.
While on a secure page, the lock icon on the bottom of Web browsers such as Netscape Navigator and Microsoft Internet Explorer becomes locked, as opposed to un-locked, or open, when a User is surfing. While WAVE uses encryption to protect sensitive information online, WAVE also uses its best efforts to protect user-information off-line. User information, not just the sensitive information mentioned above, is restricted in WAVE offices. Only employees who need the information to perform a specific job (for example, our billing clerk or a customer service representative) are granted access to personally identifiable information. SUPPLEMENTATION OF INFORMATION
In order for the WAVE website to properly fulfill its obligation to customers, it may be necessary to supplement the information WAVE receives with information from 3rd party sources.
WAVE may send new members a welcoming email to verify password and username. Established members may occasionally receive information on products, services, special deals, and a newsletter. Out of respect for the privacy of our Users WAVE presents the option to not receive these types of communications. Please see our choice and opt-out below.
CORRECTION/UPDATING PERSONAL INFORMATION:
If a users personally identifiable information changes (such as your zip code), or if a user no longer desires our service, we will endeavor to provide a way to correct, update or remove that user's personal data provided to us. This can usually be done at the member information page or by emailing our Customer Support. Headquarters:
WAVE Users are given the opportunity to opt-out of having their information used for purposes not directly related to the WAVE site at the point where WAVE asks for information. For example, our order form has an opt-out mechanism so Users who do not wish to receive marketing material may keep their email address off of our lists. Users who no longer wish to receive our newsletter or promotional materials from our partners may opt-out of receiving these communications by replying to unsubscribe in the subject line in the email or email us at firstname.lastname@example.org
